What are you supposed to give for gratuity to a massage therapist?

I got a 60 minutes massage yesterday, and it cost me 94.50 Cdn. I left $5.00 gratuity because I had no clue what your supposed to tip. It was my first massage. What do you regularly give them? What are you supposed to give for gratuity to a massage therapist?
It's good to ask before the service so there's no question, because the answer varies on the type of service and where it was given. I would rather answer the question so the client isn't uncomfortable wondering about it.

The general rule is to tip for massage in a spa, but not in a medical setting. In Canada massage therapists must complete a lengthy education and are considered medical paraprofessionals. You do not tip medical staff of any sort.

You also aren't expected to tip business owners, just contractors and employees.

The appropriate amount to tip in spa settings depends on the quality of the service. I've seen answers from 10% to 25% of the price of the massage. What are you supposed to give for gratuity to a massage therapist?
Think about it this way: You tip a server in a restaurant, and the general accepted amount is 20% of the total of your bill, give or take, depending on the quality of their service, etc. Now consider how much more a massage therapist is doing for you for the time you are with them, also the fact that they are attending only to you and not to four or five other parties at the same time as a server would, and how much more educated a massage therapist is than a restaurant server. Considering all of that, and assuming that they treat you in a hospitable manner and deliver excellent customer service as you'd expect a server to do, doesn't the massage therapist deserve at least the 20% you'd give to a server in a restaurant?
